Junior Group Golf at Commons Park
This class is a walk-up which means golfers are not required to attend each week but if they are able to attend weekly and practice in between lessons, they will progress quicker.   This form is for families that have been invited to join the class and have completed the liability waiver.  If you would like an invitation please call 708-584-0887.

    Attendance Submission

    There is enough room to enter multiple names of golfers in the space, no need to submit one form per golfer.
